Continuum offers a full range of assessment and treatment services within the home
environment or current place of residence (hospital, family residence, convalescent home)
to ensure early intervention following accident or injury and to provide the necessary rehabilitation
services to assist individuals in reaching their optimum level of functioning.

Assessments take into account the current living environment and any issues for rehabilitation
related to safety, changes in cognition or behaviour, nutritional status, environmental accessibility,
family involvement/dynamics, language communication barriers and cultural preferences.

Treatment services are provided within the home environment and involve communication with other treating professionals, caregivers, and family to ensure a collaborative approach to rehabilitation. In-home rehabilitation encourages the involvement of family / caregivers and facilitates the functional carry-over of skills to the ‘real life’ environment. Therapy services within the home also allow timely access to rehabilitation to those individuals who, based on their injuries, cannot access outpatient services.
