Returning home from the hospital following a significant injury or illness can be overwhelming, especially when it comes to meeting care needs, organizing equipment, and addressing environmental accessibility issues. Continuum therapists are trained to coordinate this process with the hospital, care providers, the client/family, and the referral source to ensure that adequate measures are in place to facilitate a smooth transition back to the home environment. Therapists assess the need for personal care assistance (completing pre-discharge Form 1, Assessment of Attendant Care Needs) and assistance within the home environment, and recommend equipment to address safety and accessibility concerns. |
